Medicare Prescription Drug Coverage

Understand your Part D options.

Your prescriptions, pharmacy, and coverage needs are personal. Medicare Part D helps pay for prescription drugs, but the details differ from plan to plan. Start with the medications you take and compare the full picture.

Check your medications Compare pharmacy costs Review coverage each year

Two common ways to get drug coverage.

Part D is offered through Medicare-approved private plans. The right enrollment path depends on your existing coverage.

Stand-alone prescription drug plan

A Prescription Drug Plan, or PDP, adds drug coverage to Original Medicare and certain other Medicare arrangements. Your medical and prescription coverage remain separate.

Medicare Advantage with drug coverage

Many Medicare Advantage plans include Part D along with medical benefits. Check the plan’s medications, pharmacies, and medical provider network together.

Four details worth comparing.

A low monthly premium does not tell you the total cost.

Your medication list

Confirm each prescription’s exact name, strength, dosage, and quantity. Check whether the plan covers it and which cost-sharing tier applies.

Your pharmacy choices

Review preferred and standard network pharmacies, along with mail-order options. The same medication may cost different amounts at different pharmacies.

Your total expenses

Consider premiums, deductibles, copayments, and coinsurance. Higher-income beneficiaries may also owe an income-related premium adjustment.

Your coverage requirements

Check prior authorization, step therapy, quantity limits, and other rules that could affect how you receive a medication.

Review official Part D coverage guidance

Make the terminology easier to follow.

Select a topic for a straightforward explanation.

What is a formulary?

A formulary is the plan’s list of covered prescription drugs. Plans cover a range of medications, but their lists are not identical. Check your specific prescriptions before enrolling.

How do drug tiers work?

Plans often group medications into tiers with different cost-sharing. A drug’s tier can affect what you pay, and tier structures vary by plan.

What are prior authorization, step therapy, and quantity limits?

Prior authorization requires plan approval before coverage for a particular prescription. Step therapy generally requires trying another covered medication first. Quantity limits restrict how much the plan covers over a stated period.

Your prescriber can help address these requirements or support an exception request when appropriate.

What if my medication is not covered?

Ask your prescriber about covered alternatives and whether an exception request is appropriate. Plans have coverage determination and appeal procedures; approval is not automatic.

Does Part D cover every drug or vaccine?

No. Coverage depends on Medicare rules and the plan. Some medications and vaccines are covered under Part B rather than Part D. Most over-the-counter drugs and certain other products are excluded from the basic Part D benefit.

Confirm coverage for the specific medication and how it will be administered.

Should I review my plan every year?

Yes. Premiums, formularies, pharmacy networks, and cost-sharing can change. Your prescriptions may change too. Review the plan’s Annual Notice of Change and compare options for the coverage year you are considering.

Understand costs and available assistance.

Use the plan’s current documents and official Medicare guidance for the applicable coverage year.

Annual drug spending limit

Part D has an annual out-of-pocket limit for covered drugs. Premiums and noncovered drugs do not count toward that limit. The amount can change each year.

Prescription Payment Plan

This option spreads eligible out-of-pocket prescription costs across the calendar year. It changes when you pay, rather than lowering the total cost.
